I had actually found a website that creates the POA form for you with the details you enter and filled it out and signed in front of a notary and two witnesses. I don’t know if it’s gonna work but it was the safest bet. Thank you for trying to help though!

I’m aware of this form but it is not relevant to my issue because it is for vehicle purchasing purposes, not license obtaining purposes.
