You are incorrect about it being a placebo effect in regards to just honey and lemon. Honey actually breaks down proteins ( mucus is a protein). But this only works when the temperature of the mix stays under 140f and the components stay intact. For anyone adding really hot water they are destroying it and for them it is a placebo effect.
So done correctly it is very effective at killing bacteria and breaking down mucus Done incorrectly ( as most people do) it’s just a tasty drink
