I think alcohol and other mind numbing drugs existed back then, as well as a solid wooden club to the back of patients head.

As the son of a retired facial plastic surgeon, can confirm this. But that's one of many ways. I didn't realize until teenage years it was unusual to watch surgery videos right before dinner.
