I was born in 79 and am applying for a passport for the first time. My original birth certificate, the one I’ve used for everything I ever needed something like that for doesn’t have my parents listed on it.
It’s only got my name, sex, place of birth, and date of birth. It’s still got the original seal.
I applied for my passport today and the courthouse where I live now accepted it, but it’s got me a little stressed that it doesn’t look anything like a modern birth certificate.

You can always go back to the town's Vital Statistics department and get a new one made.
