I just moved to Baltimore from another county. My polling place is an hour away. Is there a way I can change my polling place same day so I can vote in Baltimore?

Yep. You need to go to your new polling place. Use the 'where do I vote' link on the State Board of Elections website.

You'll need some proof of residence, and you'll be voting a provisional ballot.
