Columbia Associates in Psychiatry has a MD office - I go to a VA location for Spravato (nasal ketamine) and I just had to get it pre-authorized by my insurance.

Although fair warning, the staff are all really nice, but trying to get in touch with their front office can be a PITA. You might have to call a few times.
