Most of the other answers on here highlight the general lack of understanding of how nuclear weapons are used. In no scenario (outside of movies and video games) are nuclear powers lobbing nukes at each other's countries.

Nuclear weapons are wasted by attacking civilian populations, rather they are used on military forces on or approaching the front line, logistical hubs near that front lines, and forward staging areas.

As for the question about fallout and nuclear winter...

In the 21st century we now know that the idea of MAD or nuclear winter is far from settled science. This post over in r/AskHistorians gives a great breakdown on the history and difficulty of figuring out whether these things are even possible.
